Thank you to everyone who attended our Membership Event!
The gathering was a fantastic opportunity to celebrate the achievements and innovations across Primary Care Networks and healthcare teams.
Attendees shared best practices, fostered collaboration, and discussed future initiatives to enhance primary care and community healthcare. The event spotlighted key accomplishments, innovative practices, and solution-focused approaches that have contributed to success in the field.

Speakers
Dr Ben Sharland - Welcome and Introductions
Ben is the Deputy Chair for the GP Alliance representing practices in the East. He remains a full-time GP partner in Bournemouth. He has held a number of roles within Dorset CCG supporting Primary Care development and currently chairs the Primary Care Strategy Implementation Group. He is also the Clinical Director for Central Bournemouth Primary Care network.
Dr Sarah Kay
Dr Sarah Kay has worked for the last three years as the GP Clinical Lead for Quality and Patient safety at NHS Dorset as part of the Patient Safety Team.
Her other portfolio roles include working as a salaried GP in Weymouth, GP appraiser, Medical Examiner for West Dorset, and as a Salaried GP for Dorset Healthcare at Bridport Community Hospital. She also does some freelance medical education work.
She has previously had roles working in quality improvement, and health promotion and has been based in Dorset since 2007.
Kyle Ellis - Morris Crocker Accountants
With over a decade of experience working in accounts and taxation in the healthcare industry, Kyles main focuses are Primary Care Networks (PCNs), Federations, and corporate medical organisations. Kyle enjoys providing strategic and tailored solutions to meet the ever changing needs of this sector. He joined Morris Crocker in 2020.
Sam Taylor - Morris Crocker Accountants
Sam is a senior manager in Morris Crocker's very experienced Medical team, having joined in 2013, advising medical practitioners in all areas of their financial affairs including partnership accounts, superannuation, management accounting and bookkeeping.
Morgan Daly - One Care
Morgan is Deputy Director for Strategy and Digital Transformation at One Care (BNSSG). He spends his time supporting Practices with work including trialling emerging technology; AI-based tech; supporting Practices to work more effectively (and digitally) with care homes; and supporting promotion of the NHS app. His background is in public health, the voluntary sector and NHS digital transformation. He believes that digital transformation is about people first and technology second.
Rhys Lewis - One Care
Rhys Lewis is the Executive Director for Digital and Business Intelligence at One Care (BNSSG), leading operational and transformational digital and data-driven initiatives to support general practice. He collaborates with GP practices, system partners, and industry to develop innovative digital tools, streamline workflows, and enhance patient care. With a strong focus on AI, automation, and interoperability, Rhys is passionate about using technology to drive efficiency, sustainability, and better outcomes.

PCN Successes- Hypertension Pathways & Treatment Centre- Emma Prince/Dan Beale/Sam Stanley – Bournemouth East Collaborative PCN
Two of Bournemouth East Collaborative PCN's main priorities recently have been the improvement of care for patients with high blood pressure (diagnosed or not), and the establishment of a PCN Treatment Centre to provide additional appointments and support to practices. The Treatment Centre is based at Shelley Manor in a separate area that is leased to the PCN. It provides both GP, ANP, Nurse, HCA and MSK appointments, as well as providing the base for our PCN Mental Health service (including MH Triage Practitioners). The hypertension service utilises both the Viso app and a PCN designed pathway via Systm1 and is delivered largely by PCN HCAs and the PCN Pharmacy team, and has seen a dramatic improvement in management of hypertension moving from a position of one of the worst performing PCNs in Dorset to being the most improvement in terms of percentage of hypertension patients treated to target.

Using the evening service to create peer support opportunities for population health management and empowering patients to take control of their health inequalities.
The AdvantAGE Project - Health & Social Care Team Holly White – Christchurch PCN
The "AdvantAGE" Project is an initiative led by Christchurch PCN Health & Social Care Team, aimed at proactively identifying our most vulnerable patients who may not be accessing the care and support that they need in later life. As a pilot, we began with the smallest surgery within the PCN, focusing initially on patients aged 90 and older. This has now been rolled out across all four of our practices. By using a tailored approach to anticipate the needs of the older population, we are taking proactive steps to provide personalised care that enhances their wellbeing and addresses potential health and social care challenges before they reach a crisis. Our multi-faceted approach includes preventative strategies designed to support the health and independence of older patients. We strive to work in a person-centred and proactive manner to ensure that our elderly population can "age well.

Community Switch -Unlocking the potential of in-community health care and wellbeing support Jo Morris – Cranborne Practice/Crane Valley PCN / Julie Saunders – OMNIsector
Community SWITCH is a programme of self-help and signposting services. However, what makes SWITCH unique is that social prescribers and Patient Participation Group members have identified and reviewed the resources within the Crane Valley network. This ensures that Community SWITCH caters to the needs of vulnerable populations such as older people, financially vulnerable individuals, and those with disabilities or living with chronic illnesses. Tailoring assistance to meet the specific requirements of these groups ensures that no one is left behind in their pursuit of good health.
